For the Providing and installation of digital audio devices, Action for Humanity will contract with suppliers

providing and installing a digital hearing aid programmed according to the patient’s audiogram by doctor’s consultation, and conducting an audiogram. For children 6- 18 age in DZ

The suppliers can get the documents by visiting AFH offices:

Raqqa office (Raqqa city, Al-Tass-hih Neighbourhood – next to Al-Rasheed compound.),

DZ office: western countryside – Safera village – Toiler Bassam

Bidding condtion

·       Bidder must fill the RFQ form.

·       Bidder must sign the technical conditions form which been shared by SYD.

·        A catalogue of the submitted items must be provided during the submission, and a real sample will be requested before contracting with the winning company

·       Bidder must include all documents related to the company registration and profile.

·       Bidder must include ID copy of company owner or the authorised IDs copy.

لتوفير وتركيب أجهزة سمعية رقمية ، ستتعاقد منظمة أكشن فور هيومانيتي مع مزودين لتقديم وتركيب اجهزة سمعية رقمية حسب المخطط السمعي للمريض مع استشارة الطبيب المختص وإجراء التخطيط السمعي .للأطفال 6 -18 سنة في ديرالزور

يمكن الحصول على الاوراق عن طريق زيارة مكاتب منظمة العمل من اجل الانسانية

الرقة : الرقة جمعية الرشيد, غرب مشفى المواساة, جنوب مولدة الرشيد

·       على كل عارض متقدم ملىء نموذج عرض السعر

·       توقيع استمارة الشروط الفنية-وارفاق كتلوك للمادة المقدمة

·       يجب توفير كاتلوج للمادة المقدمة  اثناء التقديم وسيتم طلب عينة حقيقة قبل التعاقد مع الشركة الرابحة

·     ختم وتوقيع كامل المرفقات وارفاق جميع المستندات المتعلقة بشركته متضمنةً وثائق التسجيل الشركة والملف الشخصي

·      ارفاق نسخة عن البطاقة الشخصية للعارض أو للمفوضين بالنيابة عنه.
